The 4Leaf Survey (12 questions, 2 minutes) estimates the percentage of your daily calories that are from the healthiest foods: Whole Plants.
This nutrition assessment has been well researched and validated, IF you are honest and estimate the number of your servings accurately.
Please Note: 1 serving = 1/4 plate
Also, be aware that some questions differ in regard to the number of servings being "per DAY" OR "per WEEK."
You do NOT need to keep a food diary for a week before taking The 4Leaf Survey.
Simply recalling your "typical meal, snack, and beverage pattern" in response to the basic questions will suffice.
