Lifestyle Professionals

​OptiHealth Lifestyle Centers provide Lifestyle Medicine Support Services for patients who want to achieve the best possible treatment outcomes.
Medical  Professionals refer their patients to our Network of Lifestyle Professionals to help track and motivate patients in achieving optimal health.
It all starts with a free* OptiHealth Lifestyle Risk Assessment.
Certified Lifestyle Medicine Technicians conduct free Lifestyle Risk Assessments on referred patients based on their age, gender, and health status. The patient receives a Lifestyle Score and a medically appropriate Lifestyle Rx, and the referring physician receives a 1-page Progress Report with each follow-up Assessments. The initial Assessment is free. Follow-up Assessments cost only $10 each and are recommended every 1-3 months, depending on the patient's health and lifestyle goals. Each OptiHealth Lifestyle Assessment takes only 20-30 minutes, including:
  • Biometrics: Body Mass Index (BMI) and Waist Circumference.
  • Interview: An in-depth conversation covering 12 of the most important lifestyle risk factors.
  • Goals: A candid discussion about the results in order to set some realistic goals for developing a healthier lifestyle.
  • Lifestyle Rx: A personalized lifestyle prescription that is very "do-able," yet effective in achieving optimal health.
As a certified Lifestyle Medicine Technician, you can establish an OptiHealth Lifestyle Center at your local church to conduct Lifestyle Risk Assessments for patients referred by their doctors and as a valuable community resource for the general public. For more information, simply Register with us today.
